There is incongruity...

We've both seen someone we love, through the final stages of this hideous disease....through the frailty and emaciation. The pictures don't match the reports of how his illness has progressed...treatments etc. I suspect many such photographs have been taken through the years to pacify a regime that requires visual proof of their leader, to ensure calm and order.
