Best if you all contact your Broker and get your Confirmation number on the number of shares in your account from them.

Having what ever it is they must give you, you can then access "Equity Transfer & Trust Company" via the internet and access the Voting procedure through them. If not suceeding ask Equity what you need to get from your Broker and then get what ever and go back to the site where you can make your vate count!

As for getting the Original Info package out to you, that is Equtiy's responsibility, either to your Broker (who then forwards it o you) or to you directly.

Note that many such Annual Reports and Proxy Vote forms that go through your Broker first are often very late getting to you supposing they arrive at your mailing address in time at all, thus call your Broker to see how you initially set up your account with them (direct ownership of your assets - thus listed directly with Equity, or ownership thorugh your Brokers account - thus listed with Equity through your Broker).
