To add to your points Tada, let us not forget that most countries do not follow the same rigorous standard of care as the USA or Canada.  

So the uniformity imposed by this trial on all participating patients by itself will improve the quality of standard care that each of them are now receiving.  

Which would naturally skew the Placebo MACE rates in our trial lower than trial design or as implied by similar trials that took place years ago.
