Apabetalone "replace" statins? No way, in my opinion. LDL-cholesterol is cemented as a cardiovascular risk factor and there is no evidence thus far that apabetalone lowers LDL-C. A more likely threat to replace and/or be used in combination with statins (as an alternative to high dose statins) are other new agents that lower LDL-C, like PCSK9 antibodies, inclisiran PCSK9 siRNA, or bempedoic acid.

Really the only overlap between apabetalone and statins is that some statins lower hsCRP and there is some evidence that apabetalone does this too (hopefully BETonMACE confirms this). Bempedoic acid, but not the PCSK9 drugs, also lowers hsCRP. For those who can't tolerate statins to achieve their LDL-C goals, there are current and soon to be available statin alternatives to lower LDL-C. The anti-statin movement has as much basis and respect as the anti-vax movement. Both have followers, but the science just doesn't back it up.
