"Underway" is a vague term. In my mind, a trial doesn't start until recruitment begins. However, others may have a different interpretation, which is fine. There is a lot of work that goes into designing trials, getting study sites and lead investigators lines up, and getting the trial approved and listed on ClinicalTrials.gov. All these things have been "underway" for some time and are necessary to sort out before recruitment/dosing begins. So it all depends on how one defines "underway."
