Don't wait for the proxy form, if you don't have it, it has probably been lost or been thrown out.

Call the company where your shares are held and they can give you this informaiton.

For example, my shares are with Q-Trade.  I called their customer service and asked for the control number for my Noront shares in each account where I hold them.  To be clear, this means there is a different control number for each account where shares are held (i.e. RRSP, TFSA, Non-Registed, etc.)  

They e-mailed this information to me within 24 hrs.  You can then vote the shares by phone or online using the control numbers provided
