Wolf, great information as always. I have been going through a form of certificate Russian roulette lately(not ptsc related) and naming names or locating shares is not the easiest thing to do especially since there are many different transfer houses and especially if shares are in certificate form.
Shares held in street name through a brokerage account or shares held in certificate form (as you correctly point out), (certificate form due to cost and turnover is the old format, hardly used anymore except mainly by individuals who hold stocks for dividend purposes and have no intent to sell for a lengthy time period). For purposes of corporate voting however proxies and other corporate material is usually sent out by the transfer house who keeps track of street name accounts.
However, I am aware of many instances where not all shareholders have control of all their street shares in accounts under their name. Control can be under the names of relatives and or children, trusts, trustees etc... Therefore proxies are often voted by others then the named shareholder by right of power of attorney. Addresses for share ownership also may vary. Obtaining shareholder list is not always accurate as to actual ownership.
