Thank you for providing us your signed opt-in agreement. We have reviewed your agreement and can confirm that you have successfully “opt-ed” in to be represented by the Ad-hoc Committee of Crystallex shareholders.

As per paragraph 7 of the agreement, please let us know if there are any changes with respect to how many Crystallex shares you own or the capacity in which you hold them.
